VL2 form - Renewal Form

VL5 Form - Deactivation

This is a permanent deactivation of the vehicle licence. Once surrendered a licence can never be restored, renewed or used ever again. This form should be completed with all relevant details and submitted to the National Transport Authority. Properly completed requests will be processed within two working days of receipt. SPSV Test application form

It is a requirement when you arrive at the test centre to take your chosen test, to have with you the valid form of identification you used at the time of booking your appointment, e.g. If you have specified your driving licence at the time of booking, you must have the same ID to present when you arrive at the test centre.
